- Mother’s health before and during pregnancy
- The story of your child’s birth
- Evaluation of events leading to health or developmental issues
- Immunization history
- Toxic exposure
- Diet (mother’s if breast feeding)
- History of any trauma

- Physical and osteopathic structural exam
- Evaluation of walking, hands and knees crawling, belly crawling, heel-to-toe walking, drawing ability and visual tracking
